The Bracha Rishona on Tomato Rice Soup is as follows, Borei Pri Hoadama or Mezonos, and the Bracha Acharona is Borei Nefashos.

If the main intention is to eat the rice then one would recite a Mezonos, however if one's main intention is to eat the tomato, one would recite a Hoadama.

Baruch Atah Adonai, Eloheinu Melech Haolam, Borei Pri Hoadama.

or

Baruch Atah Adonai, Eloheinu Melech Haolam, Borei Minei Mezonos.

בָּרוּךְ אַתָּה ה' אֱלֹהֵינוּ מֶלֶךְ הָעוֹלָם, בּוֹרֵא נְפָשׁוֹת רַבּוֹת וְחֶסְרוֹנָן עַל כָּל מַה שֶּׁבָּרָאתָ לְהַחֲיוֹת בָּהֶם נֶפֶשׁ כָּל חַי, בָּרוּךְ חֵי הָעוֹלָמִים.

English

Blessed are You, Hashem our God, King of the universe, Who creates numerous living things with their deficiencies; for all that You have created with which to maintain the life of every being. Blessed is He, the life of the worlds.

Transliterated

Baruch Atah Adonai Eloheinu Melech Haolam, Borei Nefashos Rabos V'chesronan Al Kol Ma Shebarasa L'hachayos Bahem Nefesh Kol Chai, Baruch Chei Haolamim.
